Correlation Between Erectile Dysfunction And LUTS The AUA Lecture

UroToday.com - Dr. Kevin McVary (Chicago) presented the AUA lecture. The incidence of ED increases to over 60% by age 60. He showed data that ED increases with increasing severity of LUTS, independent of age. Across all age groups, increasing LUTS correlated with worse ED even after adjusting for other factors such as diabetes mellitus. This suggests a common pathophysiology or causal relationship between ED and LUTS.
In the MTOPS data, sexual function domains were stratified against AUA symptom indices and all sexual function domains correlated with LUTS. Prostate volume also correlated with ED as increasing size and worsening ED were linked. Several theories may explain these findings. Nitros oxide (NO) can be reduced by smoking, diabetes, etc. and result in prostatic hyperplasia that contributes to LUTS. In a canine prostate model, exposure to NO results in increased relaxation of smooth muscle, consistent with the hypothesis. Dr. McVary’s research has shown decreased NO as men age. The enzyme PDE-5 in the prostate and bladder is increased and may contribute to ED. Experimental models of PDE-5 inhibitors result in increased smooth muscle relaxation.
He spoke about using the PDE-5 inhibitor concept to treat LUTS. A placebo run-in phase is important in LUTS trials. Sildenafil was tested in a combined ED/LUTS trial, and LUTS scores improved by over 6 points. Both obstructive and irritative scores improved. He cited Dr. Steif’s vardinafil study that also demonstrated an improvement in LUTS. Despite symptom improvement, Dr. McVary did not find any improvement in urinary flow rate.

Another theory is that increased autonomic activity correlates to ED and LUTS. He presented data in patients to support this correlation. A causal relationship was not demonstrated. Increased sympathetic tone from obesity, physical inactivity and other causes lead to effects on erectile function and LUTS.

New Penile Pump Could Improve Patient Satisfaction, Confidence

Erectile dysfunction (ED) has a dramatic impact on quality of life. Treatments for ED continue to improve, and penile prostheses have one of the highest patient satisfaction rates. In the April 2008 issue of Urologic Nursing, Susanne A. Quallich, Dana A. Ohl and Rodney L. Dunn survey urologic health care practitioners to find the penile prosthesis pump that is easiest to use and teach to patients.

Smoking cigarettes causes cancer and increases the risk of cardiovascular and other diseases, but there’s even more bad news for men who smoke. They have an increased risk of experiencing erectile dysfunction, and the more cigarettes smoked, the greater the risk, according to a study by Tulane University researchers published in the American Journal of Epidemiology.
Although erectile dysfunction, the consistent inability to achieve or maintain an erection sufficient for satisfactory sexual performance, is not a life-threatening condition, it compromises well-being and quality of life. The study results suggest that smoking prevention is an important approach for reducing the risk of erectile dysfunction.
A team of researchers led by Jiang He, professor and chair of epidemiology at the Tulane University School of Public Health and Tropical Medicine, examined the association between cigarette smoking and erectile dysfunction in a 2000-2001 study in China involving 7,684 men. The researchers used questionnaires to assess the status of cigarette smoking and erectile dysfunction.
"The association between cigarette smoking and ED was found in earlier studies," says He. "However, most of those studies were conducted in patients with hypertension, diabetes and cardiovascular disease. What distinguishes this study is that it is the first to find this association among healthy men."

Those surveyed by the team were men between the ages of 35 to 74 who did not have vascular disease. Overall, an estimated 22.7 percent of erectile dysfunction cases among Chinese men might be attributable to cigarette smoking, according to the study.

More than 18 million American men — nearly one in five — experience erectile dysfunction, according to a study this year from the Bloomberg School of Public Health at Johns Hopkins University.
"There have been studies done in this country that show the same association between cigarette smoking and ED, but those were conducted in patients with diabetes, hypertension or cardiovascular disease. Our study shows that even in healthy men the association is very strong," He concludes.
Raising further caution flags about the dangers of starting to smoke cigarettes, the researchers found that even when cigarette smokers quit, their risk of developing erectile dysfunction did not decrease. The risk is statistically about the same for former cigarette smokers as for current cigarette smokers.
"This study really has a strong message for young men," He says. "It may get their attention if they know that smoking is associated with ED even in the healthy population. So the message is: Don’t start."

Mapping of prostate cancer genes opens the door to new treatments

Genetic changes during the initiation and progression of prostate cancer have eluded scientists to date. Now for the first time researchers have identified a specific gene expression profile of prostate cancer stem cells, with important implications for future treatments.
The findings, published in BioMed Central’s open access journal Genome Biology, revealed 581 genes that are differentially expressed in certain prostate cancer cells, highlighting several pathways important in the cancer stem-cells biology, and offering targets for new chemopreventative and chemotherapeutic approaches.
The cells in the study represent less than 0.1% of prostate cancer tumors, and have properties that mark them out as cancer stem cells. The cells renew themselves, are highly invasive, and have a longer lifetime than normal stem cells. They also feature a primitive epithelial phenotype and can differentiate to recapitulate phenotypes seen in prostate tumors. The cells are found in all stages and types of prostate cancer.
Expression profiling of prostate cancers typically uses tumor cell mass samples to identify individual genes. In this study, researchers harnessed advances in microarray and target labelling technologies to produce a functionally annotated expression profile of these prostate cancer stem cells.
The team, from the YCR Cancer Research Unit at the University of York and Pro-cure Therapeutics Ltd, created a malignant stem cell signature by combining genes significantly overexpressed in stem cells with those significantly overexpressed in malignant stem cells. Quantitative RT-PCR, flow cytometry and immunocytochemistry were used to validate the gene expression changes.
Genes associated with inflammation were prominent in the cancer stem cell expression profile. Potential therapeutic target NF?is known to promote cell survival. The researchers showed that an NF?inhibitor triggered programmed cell death in cancer stem cells, but spared normal stem cells. This provides a potential therapeutic target for this rare group of cells, which are unlikely to be affected by current chemotherapy regimens.

Men at increased risk of death from pneumonia compared to women

Men who come to the hospital with pneumonia generally are sicker than women and have a higher risk of dying over the next year, despite aggressive medical care, according to a study being presented Tuesday, May 20, at the 104th International Conference of the American Thoracic Society. Scientific sessions are scheduled May 16 to 21 in Toronto.
"It is well known that women live longer than men. We have always assumed that these differences occur because men engage in riskier behaviors and have a greater burden of chronic diseases," said Sachin Yende, M.D., study co-author and assistant professor in the Department of Critical Care Medicine, University of Pittsburgh School of Medicine. "Our study showed that men were more likely to die up to a year after pneumonia, despite adjusting for health behaviors and chronic conditions. Further, our findings indicate this may be linked to differences in immune response."
The University of Pittsburgh researchers evaluated data from 1,136 men and 1,047 women with symptoms of pneumonia who were treated at 28 hospital emergency departments in the United States.
On average, men arrived at the emergency departments with poorer vital signs, were more likely to be smokers and had a greater variety of complicating health conditions. "The gender disparity on aggressiveness of hospital care is appropriate, given that men tended to be significantly sicker than women," said Michael Reade, D.Phil., first author, former fellow in Pitt’s Department of Critical Care Medicine, and assistant professor at the University of Melbourne, Australia. Investigators adjusted results for age, race, tobacco use, other demographic characteristics, chronic health conditions, health behaviors and levels of treatment. Using the patient data, Drs. Reade, Yende and their colleagues next examined a series of molecules important to the body’s immune response to infection, finding significant differences between men and women in levels of tumor necrosis factor, interleukin-6, interleukin-10, antithrombin III, Factor IX, plasminogen activator inhibitor-1 and D-dimer. By comparison between the genders, some concentrations were lower, while others were higher.
"No one had shown this before," said Dr. Yende. "So these differences in immune response could explain at least some of the differences in survival."
Gender differences in immune response to infection are an area of intensive research, with investigations into the role of X chromosomes, which encode genes for several important immune system mediators. "Some investigators have suggested that the immune response may differ because women have two copies of the X chromosome," said Dr. Yende. "Sex hormones are another area of inquiry. It is tempting to speculate that in the future, medical treatment of pneumonia may be different for men and women."
